Identificando o risco de suicídio de pacientes em unidades de internação clínica por meio de uma aplicação móvel: resultados clínicos

Introdução: O suicídio é uma calamidade que afeta a saúde pública globalmente, tornando-se necessárias ferramentas para ajudar os profissionais na detecção do risco deste fenômeno. Objetivo: Apresentar os resultados da análise dos dados clínicos obtidos por meio da aplicação Detect-S e da MINI (Mini-International Neuropsychiatric Interview) em pacientes internados em hospitais de Porto Alegre/RS. Métodos: O estudo de abordagem transversal, qualitativa e quantitativa ocorreu no Hospital Santa Rita (HSR) e no Hospital Materno Infantil Presidente Vargas (HMIPV), de janeiro de 2018 a fevereiro de 2019. A pesquisa envolveu 6 profissionais da saúde, que aplicaram ambos os instrumentos em 38 pacientes. Resultados: As seções 1 e 2 do DETECT-S mostraram uma correlação direta com os escores da MINI. O grupo com ideação suicida (C/IS) teve idade média menor (p<0,001), menos anos trabalhados (p=0,02), maior proporção de mulheres (p<0,001) e maior procedência de Porto Alegre (p=0,037). Apenas a Seção 3 não demonstrou associação significativa com a MINI, mas foi mantida no aplicativo para melhor avaliação clínica. A curva ROC, excluindo a Seção 3, mostrou que a pontuação total do DETECT-S teve boa sensibilidade (86%) e especificidade (87%), com um ponto de corte de 2,5. As avaliações do DETECT-S e da MINI mostraram uma forte concordância na identificação do risco de suicídio (p<0,001). Conclusão: O DETECT-S é uma ferramenta viável para auxiliar os profissionais de saúde na avaliação de aspectos relacionados ao suicídio.

Identifying the risk of suicide in patients in clinical inpatient units through a mobile application: clinical results

ABSTRACT


Introduction:
Suicide is a global healthcare calamity, and new tools to assess risk regarding this fenomenon are needed. Objective: To present the analysis of clinical data collected by Detect-S and MINI (Mini-International Neuropsychiatric Interview) in patients hospitalized in Porto Alegre/RS. Methods: This cross-sectional, qualitative, and quantitative study was conducted at Hospital Santa Rita (HSR) and Hospital Materno Infantil Presidente Vargas (HMIPV) from January 2018 to February 2019. The research involved six healthcare professionals who applied the DETECT-S and the MINI to 38 hospitalized patients. Results: Sections 1 and 2 of the DETECT-S showed a significant correlation with the MINI suicidality scores. The group with suicidal ideation (SI) had a lower mean age (p<0.001), fewer years of work experience (p=0.02), a higher proportion of women (p<0.001), and a greater proportion of individuals from Porto Alegre (p=0.037). Only Section 3 did not show a significant association with the MINI, but it was retained in the application for better clinical assessment. The ROC curve, excluding Section 3, showed that the total DETECT-S score had good sensitivity (86%) and specificity (87%), with a cutoff point of 2.5. The DETECT-S and MINI assessments demonstrated strong agreement in identifying suicide risk (p<0.001). Conclusion: The Detect-S is a viable and effective tool for identifying aspects related to suicidal ideation and behavior in hospitalized patients.
